BRIEF SUMMARY AND ANALYSIS Observer sent a xerox copy of the old form 164 to report a sighting of 2 Mar 68. Since the information was incomplete he was requested to complete a Form 117 on his sighting. He reported on this form that he saw the UFO on 16 Feb 68. The observer lives close to an airfield and is only 12 years old. (Data shown above was taken from the form 117.)
