11. BRIEF SUMMARY AND ANALYSIS Request for form to report sighting. Requester mailed for form from Tampa, Fla and requested that the form be sent to 4 Tulip St., Summit, New Jersey.
COMMENTS: Form NOT returned after 90 day delay. Alleged witness had requested FTD Form 164 on previous occasions and was informed that this Form was for the use of those wishing to report a sighting. It is believed in view of previous correspondence from the individual that no object was observed and that the alleged report was the means used in obtaining this form for personal use.
